With the completed rebate form you must include :



a) ORIGINAL CASH REGISTER receipt from an authorized on or offline Pioneer dealer.



b) The two complete box side PANELS that contain the UPC Barcodes.

IMPORTANT: See http://www.PioneerProDj.com for the EXACT panels that are required. (also pictured below) There will be no exception for customers sending in the wrong panels or sending only one panel.



c) One of these panels must have the Serial number stickers



Complete ALL of the form above and send the completed form via CERTIFIED mail, together with (a) the original CASH REGISTER receipt, (b) two complete box side PANELS.

Pioneer will not accept for processing incomplete forms or forms not accompanied by required items. Allow 8 weeks for delivery. The rebate is open only to residents of the United States. What is required from the box???



1st) Cut the complete quarter panel from the box. Then turn it around and cut the OTHER quarter panel.







2nd) Both quarter panels will have the UPC printed on it. Make sure the serial number sticker is on the box.







3rd) Fold the two quarter panels in half and put them in a 9x12 envelope. Put the reciept and the rebate form in the envelope also and mail it in.
