I'm assuming you're using two different soundcards for the output to your headphones and the PC Output? This is likely due to a couple of factors, one of which is a different buffer size on each audio device, the second is a lack of clock sync between the two audio devices. Instead, try using the master and monitor from the same audio hardware.
