cant get rekordbox licence key

 On website the web pages questonaire that you are required to fill in before you get a rekordbox licence. is not showing all the check boxes. so it keeps telling me ive not filled in all the fields.needs sorting out PIONEER.i tried 6 times .it only took me twice to get my driving licence.
