SP-16: Three VERY EASY improvements

The S16 is looking really good, and I am so close to buying one! 

Please consider adding these features which should be trivial to implement!

- Send MIDI program changes on MIDI tracks when a pattern starts, not when a scene is changed. This is essential for some of us who are syncing drum machines to the SP-16

-Ability to rename patterns 

-Ability to rename CC messages with the names saved with the track information! This would make it SO much easier to work with MIDI synths.

Ideally a full Instrument definitions functionality should be implemented, with definitions loaded as text files from a USB drive or set from the device, whichever is easier! This should make is very simple to work with MIDI gear: Once an instrument is defined, it is also assigned its own MIDI channel so that it can then be selected from the SP-16 for use in any track without having to set the MIDI channel and the CC#s you want to use every time...
