2 issues I can't figure out: track plays unexpectedly and files renamed

I have had this problem a few times over the years and can't seem to figure out how to fix it.  This weekend it happened 2x during live sets and I had to throw on an aux and restart rekordbox to make it stop. 

While in performance mode, a wrong keystroke sets an acapella or track playing and there's no way I can figure out how to turn it off without exiting rekordbox.  I am using a Macbook pro with the highest M1 chip and a DDJ-RZ controller.  

I will be typing in a song or searching on the laptop and something triggers the song, both times it was an acapella.  No volume control works i.e. zeroing all the faders makes no difference, it keeps playing full volume.  It's not beat-matched either, the track is just playing at regular speed which makes me think it's some sort of sampler function perhaps, right now all my sampler boxes are showing empty but I didn't check in the moment to see if that was what was happening.  

The second issue I have been having is that files keep getting renamed in rekordbox.  I'll pull up a track and it's the wrong file, often it will happen with an acapella being renamed as the actual song, for example "WAP (ACAP)" is renamed as "WAP (Dirty)" i.e. they are both songs with the same key and beats per minute.  Sometimes it happens with songs in the same key and BPM that aren't related.  It's really annoying as I will search for songs and find they no longer exist in my library, only to find that somehow they were renamed as something else.  I feel it is happening somehow while I'm using the "related/similar tracks" function as I am using this heavily while performing live to mix and mashup beats with acapellas.
