It's hard to judge based on the description alone, but you can contact an authorized service centre in your region to have your mixer inspected and/or repaired.
Whenever I put the headphone cue mix into stereo mode, it causes distortion, filtering, and reverb in the headphone mix. It sounds awful and all the bass is stripped in the cue mix (this is not a phase issue)
When the headphone cue mix is in mono split everything sounds normal, but as soon as I switch it over to stereo the audio issues begin to happen. The master output sounds fine, as well as everything else.
-The issue only occurs when the headphone cue mix is in stereo mode.
-Also, it was working fine yesterday. I don't like monitoring in mono split.
Firmware up to date, same with drivers.
This issue happens when using dj software, or during regular playback from the computer. Only effecting the headphone mix in stereo mode.
Any suggestions?

I have the same... And just the same working fine the day before...
I guess this is a hardware failure, would anyone able able to confirm this,
It seems like this is a problem with the Headphone jack (Bent). I remember standing on my cable which pulled the headphones out of the socket.
Ive also read on another post the same issues on different mixer was resolved by straightening the headphone jack.
I have tried unsuccessfully to straighten by jack but can confirm i havent got this issue if i use a different part of headphones. Now looking to replace the jack on my preferred headphones,
Yes, sounds also to me like a broken headphone jack or cable, usually you have this effect if the common (ground) contact is broken on the headphone cable / jack.
